President of Care Coordination

The President, reports to the Board and  is responsible for the overall management including:  development and  execution  of the agency budget; reporting on the status of agency finances to the  Board and to PASSE investors each month; overseeing fiscal affairs including payroll, accounts payable and receivable, inventory control, support services;  coordinating agency wide planning activities; policy development, implementation and monitoring; overseeing  human resource functions including hiring  practices and benefits administration; facilitating development,  implementation and monitoring of agency quality improvement initiatives.

The President has a dual role in that they also serve as the Vice-President for Care Coordination. In this role, the president provides leadership to 185 care coordination and support staff and is responsible for delivery of care coordination services to over 7,700 people on a 24/7 basis.
The President ensures that specialty services are coordinated and appropriately delivered by specialty providers to ensure continuity of care across all services for individuals with intellectual and developmental disabilities, and for individuals with behavioral health diagnosis.

Care coordination functions include:
  1. Health education and coaching;
  2. Coordination with other healthcare providers for diagnostics, ambulatory care, and hospital services;
  3. Assistance with social determinants of health, such as access to healthy food and exercise;
  4. Promotion of activities focused on the health of a member and their community, including without limitation outreach, quality improvement, and member panel management; and
  5. Coordination of Community-based management of medication therapy.
Job responsibilities:

Responsible for overall management of care coordination services and oversees the daily operation of all statewide care coordination functions. Included among those responsibilities are the following:  
  • Ensure that sufficient numbers of care coordinators are employed to address member’s needs and to meet Department of Human Service requirements
  • Provide direction and supervision to Care Coordination staff on a regular and as needed basis including determining work priorities, assigning work and ensuring proper completion of work assignments;
  • Provide expert level advice and guidance to staff on program and individual specific issues;
  • Develop and review annually and revise as necessary, policies, procedures for use by care coordination and other staff to assure that all state and other requirements are being met
  • Serve as co-contact along with the CEO with DHS on issues related to: care coordination services; services provided through the Home and Community Based Waiver (HCBS), Licensing and Certification issues/sanctions and; other relevant DHS regulatory and policy issues;
  • Ensure the timely and accurate tracking of data to provide status reports 
Job Qualifications:
  • A Master’s Degree in human services, social work, psychology, business administration or public administration from an accredited college or university.  A minimum of 5 years of service in administration of programs serving individuals with Developmental Disabilities or Behavioral Health diagnosis;
  • Experience as an administrator of case management services preferred;
  • Ability to communicate effectively and maintain working relationships with members and legal representatives, Board of Directors, DHS staff and providers;
  • Ability to problem solve complex administrative and clinical issues
  • Ability to analyze data for ongoing quality improvement.
