Referral Specialist

Responsibilities for this position will include tracking referrals, coordinating appointments with specialists and other providers, obtaining prior authorizations, and providing customer service to patients and clinical partners. These roles require strong organizational, communication, and computer skills to manage electronic health records and ensure smooth care transitions. 
 
Key Responsibilities:
  • Referral Coordination: Schedule appointments, track referrals in electronic health records, and ensure follow-up appointments are booked. 
  • Prior Authorizations: Obtain necessary authorizations for medications, equipment, and diagnostic imaging. 
  • Patient Communication: Interact with patients and families, providing them with information, counseling, and necessary instructions for their care. 
  • Collaboration: Work with clinical teams and external providers to optimize patient care and the overall clinic flow. 
  • Data Tracking: Monitor and report on PCMH standards, especially those related to referrals and transitions of care.
